Main tasks

  • Conducts medication history with patients, carers, or community health professionals for Hospital Pharmacists
  • Prepares a current medicines list for patients

  • Assists Hospital Pharmacists with clinical prioritisation by identifying patients at high risk of medicine-related problems
  • Records patients' laboratory data for pharmacist review
