ABOUT THIS PRODUCT
In 2007–08, the ABS conducted the fifth in the series of National Health Surveys (NHS), designed to obtain national benchmark information on a range of health related issues, and to enable trends in health to be monitored over time.
This product contains a copy of the questionnaire used in the survey, together with associated prompt cards, and a listing of all output data items available from the survey.
This package is intended to assist users of the data in making the best use of results from the survey, and should be used in conjunction with the Users' Guide publication, National Health Survey 2007-08: Users Guide (cat no. 4363.0.55.001)
Summary results from the survey are contained in National Health Survey: Summary of Results, 2007–2008 (Reissue) (cat no. 4364.0)

Note that some questions contained in the questionnaire below were sourced from question modules developed by other organisations. The questions in 'Module D - Self-assessed Health' are part of the SF-36® Health Survey question module. The questions in 'Module F - Mental Wellbeing' are from the Kessler Psychological Distress Scale (K10) question module, except for the bodily pain questions which are also part of the SF-36® Health Survey question module. Permission needs to be sought from the relevant organisation to use these questions in other surveys.
